You’re tired in a way sleep doesn’t fix. You drag through mornings, crash at 3 p.m., then find yourself wired at midnight. So you searched your symptoms, and somewhere around the third result you found it: adrenal fatigue, four tidy stages, and a description of stage three that sounded like someone had been reading your diary. Finally, an answer.
Here’s the part almost nobody tells you, and it’s more useful than the diagnosis you were about to accept.
- “Adrenal fatigue” is not a recognized medical diagnosis. A systematic review of dozens of studies found no evidence it exists as a distinct condition.
- Your exhaustion is completely real. The label is the problem, not your symptoms.
- Adrenal “support” supplements are unregulated, and some have been found to contain actual steroid hormones that can damage your hormone system.
- Those same symptoms overlap with thyroid disease, iron deficiency, sleep apnea, depression, perimenopause, and diabetes, all of which are testable and treatable.
- Addison’s disease is a real adrenal condition. It can be life-threatening. Don’t let a wellness label delay that diagnosis.

Where the “4 Stages of Adrenal Fatigue” Idea Came From
The theory goes like this. Chronic stress hammers your adrenal glands: two small organs sitting atop your kidneys that produce cortisol, your primary stress hormone. Eventually, the story says, they wear out and can’t keep up. Cortisol production falls. You crash.

The easiest win in this whole category.

The four stages are usually described as:
- Alarm: Stress hits. Cortisol surges. You feel wired, anxious, running hot.
- Resistance. You cope, but you need caffeine to start and something to wind down.
- Exhaustion: Cortisol drops. Fatigue becomes constant. Brain fog, cravings, low mood.
- Burnout / Failure: The adrenals “fail.” Total collapse.
It’s a compelling narrative. It maps neatly onto how modern exhaustion actually feels. That’s exactly why it spread.
The framework comes primarily from a 1998 book by a naturopath. It borrows loosely from the work of Hans Selye, a mid-century researcher who described how animals respond to prolonged stress: a model with three stages, describing something meaningfully different.
It did not come from clinical research demonstrating that adrenal glands become fatigued.
What the Evidence Actually Says About Adrenal Fatigue
In 2016, researchers published a systematic review examining 58 studies on the subject. Their conclusion was blunt: there is no substantiation that adrenal fatigue is an actual medical condition.
The Endocrine Society, the largest organization of hormone specialists in the world, describes it as a myth.
Here’s the anatomical problem with the theory: healthy adrenal glands don’t get tired. Cortisol production is governed by a feedback loop between your brain and your adrenals, called the HPA axis. Under sustained stress, that signaling system can shift and adapt. But the glands themselves don’t wear out and quit like an overworked engine.
What About the Cortisol Test I Was Given?
Many practitioners sell salivary cortisol panels: spit in a tube four times a day, receive a graph, receive a stage.
The trouble is that cortisol naturally swings enormously across a day. It’s influenced by sleep, exercise, food, illness, caffeine, and stress in the ten minutes before you spat in the tube. These panels have not been validated as a tool for diagnosing a condition that hasn’t been shown to exist.
A graph feels like proof. It isn’t. 💡
But Let’s Be Very Clear: Your Symptoms Are Real
Please don’t read any of the above as “it’s all in your head.” It isn’t.
Crushing fatigue is real. Brain fog is real. Waking exhausted after eight hours is real. Chronic stress genuinely damages health. It affects sleep, blood pressure, immune function, blood sugar, and mood. None of that is disputed.
What’s being disputed is one specific explanation for why you feel this way: an explanation that, if you accept it, may cost you a year of your life chasing supplements while something identifiable and fixable goes untested.
You were right that something is wrong. You were just handed the wrong map.
The Model vs. The Evidence
| What the Four-Stage Model Says | What the Evidence Supports |
|---|---|
| Adrenal glands “wear out” from stress | Healthy adrenals don’t fatigue; the HPA axis adapts |
| Stages can be identified by saliva testing | Saliva cortisol panels aren’t validated for this purpose |
| Low cortisol explains your fatigue | Genuine cortisol deficiency is adrenal insufficiency: a distinct, testable disease |
| Adrenal supplements restore function | Unregulated; some contain steroids and can suppress your hormone system |
| The label explains your symptoms | The symptoms have many possible causes: most of them testable |
| Chronic stress harms your health | ✅ This part is entirely true |
What Might Actually Be Going On
This is the section worth bringing to your doctor. Persistent, unexplained fatigue has a well-established list of causes:
- Hypothyroidism: an underactive thyroid. Fatigue, cold intolerance, weight gain, brain fog. Diagnosed with a simple blood test.
- Iron deficiency or anemia: extremely common in menstruating women. Fatigue, weakness, breathlessness.
- Sleep apnea. You may be sleeping eight hours and getting almost no restorative sleep. Frequently missed in women.
- Depression and anxiety, both cause profound physical fatigue, not just low mood.
- Perimenopause: can begin in your late thirties and disrupt sleep, energy, and mood for years.
- Diabetes or prediabetes: energy crashes, thirst, frequent urination.
- Vitamin D or B12 deficiency
- Celiac disease, often presents as fatigue rather than digestive symptoms.
- Long COVID or post-viral fatigue
- Medication side effects, including some blood pressure drugs and antihistamines.
- Occupational burnout: recognized by the World Health Organization as a real workplace phenomenon.
Notice something? Nearly every item on this list is testable. Most are treatable. None require you to buy anything from a website. 🙂
🚨 The Real Adrenal Condition You Cannot Afford to Miss
Adrenal insufficiency, including Addison’s disease: is a genuine medical condition in which the adrenal glands truly don’t produce enough cortisol. It’s uncommon, and it is serious. Untreated, it can progress to an adrenal crisis, which is life-threatening.
Symptoms overlap heavily with what gets labeled adrenal fatigue, which is precisely the danger.
See a doctor promptly if you have:
- Profound fatigue with unexplained weight loss
- Darkening of the skin, especially in creases, scars, or gums
- Strong salt cravings
- Dizziness or fainting when standing
- Low blood pressure
- Persistent nausea, vomiting, or abdominal pain
- Muscle weakness and body aches
Seek emergency care immediately for severe abdominal pain, vomiting, confusion, or collapse.
Adrenal insufficiency is diagnosed with blood testing, not a saliva kit. If any of the above sounds like you, that appointment is the single most important thing on your list this week.
⚠️ Why the Supplements Can Genuinely Hurt You
This is the part I most want you to hear.
Adrenal “glandular” supplements, typically made from ground animal adrenal tissue, are not regulated the way medications are. Independent testing has found some products containing actual steroid hormones, undisclosed on the label.
Taking corticosteroids you don’t need can suppress your HPA axis. In plain language: a supplement marketed to fix your imaginary adrenal problem can create a real one. That’s not a hypothetical risk. It’s a documented one.
Licorice root, common in adrenal blends, can raise blood pressure and lower potassium to dangerous levels.
⚠️ Important: if you’ve been taking adrenal glandulars or any product possibly containing steroids, don’t stop abruptly. Stopping suddenly can be dangerous if your body has adapted. Talk to a doctor about how to come off safely.

The genuinely good news: the things that help chronic stress and fatigue are unglamorous, free, and well supported.
- Sleep, treated as non-negotiable. Consistent schedule, dark room, no screens before bed. If you snore or wake unrefreshed, get screened for sleep apnea.
- Movement, appropriately dosed. Exercise improves fatigue, but if you’re depleted, gentle walking and strength work beat punishing HIIT sessions.
- Enough food, and enough protein. Under-eating is a wildly common and overlooked cause of exhaustion, especially among women who exercise.
- Real stress reduction. Not a supplement. Boundaries, therapy, breathing practices, time outdoors, connection with people.
- Caffeine, honestly assessed. It borrows energy from later. Sometimes that loan comes due at 3 a.m.
- A basic blood panel. Thyroid, iron, blood glucose, vitamin D, B12. Start there.
💡 Actionable Tips: Your Next Two Weeks
- Book a doctor’s appointment. Say the words: “I have persistent unexplained fatigue and I’d like it investigated.”
- Ask specifically about thyroid function, iron and ferritin, blood sugar, and vitamin D.
- Pause any adrenal supplements, but check with a doctor first before stopping, especially if you’ve taken them a while.
- Keep a two-week symptom log. Energy, sleep, mood, cycle, meals. Patterns emerge faster than you’d expect.
- Get screened for sleep apnea if you snore, wake gasping, or never feel rested.
- Eat breakfast with protein. Skipping food, then blaming your adrenals, is common.
- Protect a consistent wake time, even on weekends.
- Name your actual stressors in writing. Some are removable. You’ll be surprised which.
- Move gently and often. Walking counts. It always counted.
- Stop paying for the diagnosis. Start investing in the investigation.
You Deserve a Real Answer, Not a Label
Here’s what bothers me most about adrenal fatigue as a concept: it takes people who are genuinely suffering, hands them a satisfying story, and sells them a solution for a problem they don’t have. Meanwhile, the actual cause (the low iron, the sluggish thyroid, the untreated sleep apnea, the depression nobody named) sits there, testable, waiting.
You noticed something was wrong with your body. That instinct was correct. That instinct is worth more than any four-stage chart on the internet.
So use it. Take that instinct to a doctor who will run the tests. Ask the direct questions. Keep asking if the first answer is a shrug. You are not being dramatic and you are not imagining this.
Real exhaustion deserves real answers. Go get yours. 💙
